Update: WWE added the following details to their original story, confirming it was in fact a forklift accident that is to blame for the incident on SmackDown.

It has now been determined that the incident in question was specifically caused by a forklift driver error.
WWE.com just posted a “breaking news” announcement, revealing a forklift was responsible for the backstage incident involving Roman Reigns on this week’s episode of SmackDown.
In the final moments of this week’s SmackDown LIVE, Roman Reigns was intending to make a SummerSlam challenge from the backstage area, but The Big Dog just barely evaded sheer catastrophe when tons of steel beams came plummeting toward him.
A preliminary investigation has found the incident Tuesday night involving Roman Reigns was caused by a forklift backstage carrying lighting grids that were not properly secured.

As seen at the conclusion of this week’s show, Reigns was walking backstage to announce his SummerSlam opponent, but a bunch of equipment and scaffolding fell on him. He seemed to be OK after the incident and walked away unscathed, but it’s led to plenty of speculation as to who is behind the attack.
It’s been rumored that Daniel Bryan could be Reigns’ SummerSlam opponent, and plenty of fans speculated Buddy Murphy could be involved due to seeing him in the background of one of the camera shots. It appears both of those theories won’t pan out (for now), as it appears the attack on Reigns was just an accident.
